Survey on tandem paragliding clip-in failures

The French Free flying  Federation (Federation Francaise de vol libre) is conducting a study to better understand the causes of clip-in mistakes in tandem paragliding and explore ways to prevent them.

This questionnaire aims to gather insights from pilots around the world and compare the data with accident reports from different countries. By looking at the connections between techniques, safety protocols and incidents rates, we hope to find ways to reduce risks.

While this research is focused on tandem practice, we believe the findings could also be useful for solo pilots.

It takes about 10 minutes to complete. We’d really appreciate your time and input.

How do you do your pre-flight checks ?

Pilots may have their own approach to securing and checking their attachment points before takeoff. Some pilots check each attachment point as they clip in before moving on to the next. Others may perform their checks only once everything is clipped in. Some pilots combine both methods—checking each attachment point as they go and then performing a final check afterward. There may also be other variations

Have you ever caught a clip-in failure during your pre-flight checks? *
You may have come across a clip-in mistake during your pre-flight check at some point. These moments are valuable for analysing different practices. Fatigue, stress, and distraction are human factors that increase the likelihood of harness attachment failures, but they are not the root cause. The actual cause lies in shortcomings within the preparation and verification methods. It is these shortcomings that we aim to identify here.
